To download the Form Filer application please click here - when you clicked the link you should have been presented with a window asking to open or save, you can select either but please make a note of the location if you choose save. The setup file is called BlueberrySetup.exe double click or open this file and follow the installation prompts.

Once the program is installed you can access it from the Start menu or from the icon on your desktop. Start the program and you will see a predominantly grey screen - at the top left there is an Open icon.
You need to open the file named ' - Generic Application Form.pdf ' the program will run through a short routine then show this message

you click ok and you are on your way.

We suggest to use an ' X ' for the tick boxes and we recommend that you save often by simply clicking the blue disk icon

When you are satisfied that the form is complete you can fax or email (see details page 4 of application form) or go to our secure document upload system by clicking here.
